I'm annoyed. They have a signup page, but then you have to verify your account by contacting sales, which I refuse to do.

The only redeeming quality I can give about this tool is that they have HIPAA compliant data collection. That's not common, and it's hard to do - but these guys hide it behind a paywall that they won't price upfront - you yet again have to contact sales to purchase that add-on.

Formstack is clearly meant for enterprise customers, and that's fine - but in my experience, 'enterprise' marketed tools aren't any better than tools for your average maker, they're just upsold and overpriced like crazy.

If you need HIPAA compliant forms, use something like BlockSurvey.
